Waiver Form
(Players)

A waiver form must be completed by every skater prior to their first game.
Roster Form
(Captains)

Team captains must submit a LEGIBLE roster to the timekeeper prior to each game (see downloadable forms).

The roster form must be printed or typed, and include each player's FIRST and LAST name and JERSEY #.

If a full time player is listed on your roster but is NOT playing, his/her name must be crossed out.

Blank copies of this form will be available at the office area.
Insurance
(Captains)

Teams have the option to purchase their own liability insurance, similar to this application provided. While the NVO rink strongly encourages it, the insurance is not mandatory at this time.
